Since over 80% of seniors, people with disabilities and those recovering from hospitalization prefer to be in the comfort of their own home, we have made it our mission to empower them to remain in the homes and communities.
Our staff is available 24 hours a day, 7 days a week to provide the most convenient source of care to people in need. We provide assistance with activities of daily living such as dressing, hygiene, bathing, eating, positioning, transferring, light housekeeping, toileting and meal preparation; but also medication reminders and medical transportation.
Our caregivers are also available to provide socialization activities, reading, conversation, companionship and even daily phone calls designed to "check in". In most of our work, we assist our clients in learning to deal with their disabilities by acquiring new skills or finding creative solutions to (re)gain independence.
Scheduling is designed to accommodate your care requirements and provide you with the assistance and support you need. To do this, we also have Licensed Practical Nurses / Registered Nurses on staff at each of our seven locations. Assistance with feeding management, therapeutic exercise, long-term illness and medication management are just a few of the ways our nurses enable you stay in the comfort of your own home. These team members visit you in your home, supervise our field staff, and do initial and 180-day client assessments. We are also in agreement with Montana Medicaid Self Directed Personal Assistance Services Program, which enables you to hire your own caregivers, including friends or family.
At A Plus Health Care, we go beyond the regular duties of a home care provider and work with you to customize your plan of care with an emphasis on improving your health, wellness and independence.
